Traditional and Alternative Medicine: A Comparative Perspective

Abstract

Traditional and alternative medicine occupy an increasingly visible position in contemporary healthcare, but the two concepts should not be treated as identical. Traditional medicine refers to health systems and practices rooted in historical and cultural contexts, while alternative medicine generally refers to non-mainstream approaches used in place of conventional medical care. Complementary approaches are used alongside conventional care, and integrative healthcare seeks coordinated use of appropriate approaches. This paper compares traditional and alternative medicine across historical foundations, knowledge systems, therapeutic approaches, patient perspectives, cultural relevance, evidence, safety, regulation, accessibility, and integration into modern health systems. WHO's Global Traditional Medicine Strategy 2025–2034 emphasizes strengthening evidence, ensuring safety and quality, integrating safe and effective TCIM into health systems, and protecting cultural and Indigenous rights. NCCIH similarly emphasizes evaluating each complementary approach independently and warns against replacing proven conventional treatment with unproven alternatives. The comparative perspective shows that neither “traditional” nor “alternative” is itself a measure of effectiveness. Individual practices should be assessed according to evidence, safety, cultural context, patient needs, and the role they are intended to play in healthcare.
